Almost a third of the European population is living with a long-term condition and some, particularly the elderly, are living with two or more. We believe that Disease Management (DM)—defined as the management of the health and wellbeing of patients with long term conditions—is finally ready to make a definite breakthrough in Europe. Doing nothing is not an option, as the economics indicate that continuing today’s model of care will break the economic backs of developed countries.
Capgemini believes that we are standing at the dawn of a revolution in the way healthcare is delivered around Europe and around the world. We believe that disease management (DM) is finally ready to make a definite breakthrough in Europe. In a recent set of Capgemini workshops we took a survey of healthcare stakeholders (including patients, providers and payers) from across Europe-and 75.2 per cent agreed that disease management would be a major factor in creating stable healthcare systems in the future. Therefore Capgemini has written this document to explain:
